03. Project Rationale

3.1) Introduction to the Project:

Ashraff Memorial Hospital is the leading institution in the Deputy Provincial Director of Health Services division Kalmunai, in the coastal area of Ampara District. This hospital is serving to a population of 412542 in this division. It is a mother institution to 35 smaller institutions in the Kalmunai DPDHS area and these 35 smaller institutions are referring patients for specialized care to Ashraff Memorial Hospital Kalmunai.

Ashraff Memorial Hospital is functioning with 262 beds. During the year 2005, there were 87252 patients were treated at Out Patient Department (OPD), 36111 patients were treated at Clinics, 6650 patients were treated at Dental Unit and 19669 patients were treated inwards. There are 10 wards, ICU, Operating Theatre, Labour ward and PBU. This hospital has all supportive facilities like Blood Bank, Radiology unit, Laboratory, and ECG facilities as well. The number of patients seeking treatment at this hospital had been markedly increased in the recent past due to the peace prevailing in the country and recent Tsunami tidal wave which defunct six of close by District Hospital completely. Therefore, this institution is unable to meet the current demand for patient care services due to inadequacy of infrastructure facilities particularly inward facilities for surgical units.

In the recent past the number of Road Traffic Accidents, Bomb Blast injuries and civil war casualties seeking admission to this hospital have been remarkably increased. Therefore the demand for the well equipped Accident & Emergency Unit and Surgical Casualty wards for providing life saving measures and follow up proper patient management facilities felt mandatory.

In order to meet the growing demand our consulting are working hard to manage with limited bed strength, it was pointed out to be having the highest bed occupancy ratio of over 100% with equally highest bed turn over rate of 24% for the 3rd quarter of 2005 hospital bulletin of line ministry hospital.
The following patient care services are scheduled to commence in this institution in near future.

Even though we have approved cadre for Nursing Officers and other category of staff, non-availability of adequate quarter’s facility is the key problem to accommodate the outside nursing staff for patient care services at Ashraff Memorial Hospital, Kalmunai.

3.2) Specific Problem to be Addressed by the Project:

It is proposed to construct a four-storied building to accommodate ground floor Matron Family Suit and Sister / Master family suit, 2nd & 3rd Floor Nurses quarters with attached bathroom single units and the fourth floor Midwives quarters with attached bathroom considering the availability of land for future development of this hospital.

The propose building will be a four-storied building of 130 x 76 size with one elevator and two staircases.

3.3) Mode of Intervention in term of this Project (Justification):

At present, there are no quarter’s facilities for Nursing Officers, Midwives, Paramedics and other category of staff in this hospital. There are 54 Medical Officers, 126 Nursing Officers, 15 paramedical staffs, 65 Minor Employees and 08 Ambulance Drivers are working at present. More than 70% of the staffs are outsider and none of them are provided with a reasonable quarters/residence facilities to work mentally peacefully.

If quarter’s facilities are provided to staffs working in this hospital, they will be residing within the hospital premises and available for duty round the clock. Construction of Quarters Complex will prevent unnecessary delay in getting quality healthcare services for the people of this region. Hence this project is needed and should be given priority considering the difficulties faced by the poor and marginalized people living in this region.

Entire population of coastal area of Ampara District will benefit from the project; of which 72% are Samurdhi recipients. The entire population of this area have been displaced and resettled at one or other incidences during past and majority of this population are living in remote rural villages.
Fifty-one (51%) percentage of population is female, and majority of Nursing Officers (80%) of the hospital are female and they will be directly benefited from this project.
